Description
The management of the jaundiced patient includes radiological and endoscopic diagnostics to determine whether the abnormal liver function is secondary to an intrinsic hepatocellular condition, such as in liver cirrhosis, or to biliary obstruction. Jaundice can be the first sign of an underlying malignancy, most commonly pancreatic cancer. This is a dismal disease with a poor prognosis, and the only potentially curative treatment is surgical resection. Palliative treatment confers only a short survival benefit. Progress in surgical technique and peri-operative care has allowed a more aggressive approach and vascular reconstruction is currently performed in patients with vascular involvement. A short video of a surgical procedure will demonstrate the technique of portal vein resection and reconstruction with an interposition graft from the jugular vein in a patient with locally advanced disease. The objective of this lecture is to provide GPs and hospital clinicians with a diagnostic and therapeutic algorithm for jaundiced patients and with an update in the treatment of patients with pancreatic cancer.
The second part of the lecture is on the patient with abnormal liver function tests, differential diagnosis, investigation and treatment-including useful referral tips and tricks!
